TERMINATION OF MEMBERSHIP
RELEASE, WAIVER AND QUIT CLAIM

2. I confirm knowingly and voluntarily applying for termination of my membership/resignation from

YEARNINGS OUTSOURCING COOPERATIVE effective __________;

3. I hereby release, remise and forever discharge YEARNINGS OUTSOURCING COOPERATIVE,

its cooperators, officers and board members, regular and associate members or any other official
representative including other clients of YEARNINGS OUTSOURCING COOPERATIVE where
I was assigned, from any action, claim for sum of money, benefit or other obligations, whether
government-mandated and cooperative-driven, arising from my membership with the Cooperative;

4. I further acknowledge, declare and waive in favor of YEARNINGS OUTSOURCING

COOPERATIVE any other claims, cause of action, grievance, whatsoever against this cooperative
its cooperators, officers and board members, regular and associate member or any other official
representative with respect to any manner arising from my membership with YEARNINGS
OUTSOURCING COOPERATIVE and deployment with any other client;

5. Although none is foreseen, I consent to the presentation of this document by YEARNINGS

OUTSOURCING COOPERATIVE in any and all cases, whether civil, criminal or administrative,
that may be filed arising from my membership with and deployment to and other clients through
YEARNINGS OUTSOURCING COOPERATIVE;

6. I am executing this termination of membership and quitclaim with full understanding of the
contents hereof and all its legal implications and I acknowledge that these were explained to me in
a language that I understand and that I am freely and voluntarily executing this document;

7. And for the settlement of my claim/s with YEARNINGS OUTSOURCING COOPERATIVE and
