PROBLEM CUES/FACTS

1. Blurred Vision The patient verbalizes inability to

perform the specific activity due to blurred
vision. He said, “Hindi ko po alam, hindi ko
po makita. Kayo nalang po gumawa” and
when the student nurse asked what was
wrong with his eyes he stated that “malabo
po”.

2. Diminished Hearing As stated by the patient, “Hindi ko po

marinig”. The patient leans forward when
listening and observes lack of response,
and inability to hear spoken words.

3. Impaired Social Interaction The patient cannot maintain eye to

eye contact even in a short period of time,
and scratches his hand and back of his
neck. The client also stated that "Mahiyain
po ako", "Ayoko po sumali".
