I, PHOEBE B. SORIANO, of legal age, single, Filipino Citizen and a
resident of Brgy. Torres, Mapandan, Pangasinan, after having been duly sworn to
in accordance with law, do hereby under oath deposes and says:

1. That I am an applicant for marriage at Chiba, Japan;

2. That I am a natural born Filipino with place of birth at Mapandan

Community Hospital, Mapandan, Pangasinan;

3. That I was born on March 22, 1993 and both of my parents are
Filipino Citizens;

4. That I never acquired any other citizenship except my Filipino

citizenship;

5. That as evidence of this statement, attached herewith is a

photocopy of my Philippine Passport bearing No. P6148553B, valid
until January 21, 2031;

6. That I am executing this Affidavit to attest the truth of all the

foregoing facts and statements without any purpose of deceit or
misrepresentation and for all legal intents and purposes;
