We are students currently enrolled in Nursing Research1 (NRSGRC1) who are conducting a study
entitled: “ The Effects of the transition to online class among the Clinical Instructors in the Allied Medical
Courses of University of Baguio” This study will analyze and determine how the impacts and effects of
the transition from face-to-face classes into the virtual mode of learning has affected the performance
of the Clinical Instructors of the University of Baguio.

With this, you are encouraged to answer the question honestly. Your participation is not compulsory,
and that it is wholly voluntary. The results shall be for this research purposes only and your responses
will be treated with the utmost respect and confidentiality. Further, you may return the forms
