AFFIDAVIT
OF POTENTIAL THREAT

I, OLIVER R. LAGOSIN, of legal age, single, Filipino, with residence at

Asiaville Subd. Brgy. Masaya, Bay, Laguna after being sworn in accordance with the
law, hereby depose and state that:

1. I am currently employed at New Armstrong Security Agency as a

Security Guard;

2. I was a former member of CPP-NPA but has surrendered to the

authorities on April 2018 at Philippine Army 22 nd IB 9 Infrantry Division, Brgy.
Tinangis, Naga City, Camarines Sur and thereafter, joined the CAFGU;

3. I was being harrased by my former comrades like in one incident on

November 2019 our family house at Barangay Malbog, Pilar, Sorsogon was fired at
multiple times and left riddled with bullet holes;

4. I am included in the Order of the Battle (OB) or shoot to kill order by

the NPA since I am a surrenderee;

5. I acknowledge that as a surrenderee, the NPA is out to kill me and I

take the potential threat to my life very seriously;

6. That I am executing this affidavit to attest to the truth of the foregoing

facts and for the issuance of a Permit to Carry Firearms Outside of Residence.
