Sir:

I, Carl Dionne B. Puyat, Teacher I, and currently teaching at San Vicente

Elementary School Cabiao District, humbly applying for transfer of teaching station to
any elementary school in Schools Division Office City of Malolos for the following
reasons to wit:
1. That I am teaching in my present station for five (5) years with utmost dedication
and much achievement;
2. That I am currently enrolled in the Juris Doctor program at Bulacan State
University–College of Law and contemplating being a resident of Tikay, Malolos
City for conjugal reasons;
3. That it is my great opportunity because my present station is far from Bulacan
State University, hence time and travel expenses could be beneficial for the
upcoming transition of face-to-face classes.
4. That the approval of my transfer request will be advantageous to me as a law
student.

I hope that this request merit your favorable action.
