PETITION

Petitioner1 respectfully submits to this Honorable Court her Petition

for Commission as Notary Public for and in the City of Caloocan, Metro
Manila, together with all supporting documents required to be submitted
in accordance with the A.M. No. 02-8-13-SC, otherwise known as the 2004
Rules on Notarial Practice.2

In support thereof, Petitioner respectfully states:

1. Petitioner is a Filipino citizen, 29 years of age, born on in.

Petitioner currently resides at. She is commissioned as a Notary Public for
and in Caloocan City, Metro Manila by the Honorable Executive Judge on

2. Petitioner completed her law studies at. She passed the Bar
examinations in 2017 and was admitted as a member of the Philippine Bar

the Office of the Bar Confidant is attached hereto as Exhibit “B.”

4. Petitioner paid the Professional Tax payable for the year 2022.
A copy/screenshot of the Petitioner’s Professional Tax Receipt No. issued
on is attached hereto as Exhibit “D.”

5. Petitioner is a member of the Philippine Bar in good standing

and has no pending criminal or administrative cases filed against her. A
copy of the Certification of No Pending Administrative Case dated issued
by the Office of the Bar Confidant in favor of the petitioner is attached
hereto as Exhibit “E.” A copy of the Certification of No Pending
Administrative or Disbarment Case dated issued by the Integrated Bar of
the Philippines Commission on Bar Discipline in favor of the petitioner is
attached hereto as Exhibit “F.”

6. Petitioner has likewise no pending case, civil or criminal, filed

against her before the courts of Caloocan City, Metro Manila. A copy of the
Court Clearance dated issued by the Office of the Clerk of Court,
Metropolitan Trial Court of Caloocan City in favor of the petitioner is
attached hereto as Exhibit “G.” A copy of the Certification dated issued by
the Office of the Clerk of Court, Regional Trial Court of Caloocan City in
favor of the petitioner is attached hereto as Exhibit “H.”

7. Petitioner is a member of the Integrated Bar of the Philippines

Laguna Chapter and is up to date in the payment of her IBP dues. A copy
of the Certification issued by the Integrated Bar of the Philippines dated is
attached hereto as Exhibit “I.” In addition, a copy of the Certification dated
issued by the Integrated Bar of the Philippines – CALMANA Chapter is
attached hereto as Exhibit “J.” A screenshot of petitioner’s payment of the
IBP dues online with IBP Official Receipt No. on for the year 2022 is
attached hereto as Exhibit “K.”

8. Petitioner has likewise complied with the necessary credit units

of Mandatory Continuing Legal Education for the 7th Compliance Period
under Bar Matter No., with MCLE Compliance No.. A copy of the
Certificate of Compliance issued on by the Mandatory Continuing Legal
Education Office - Supreme Court of the Philippines is attached hereto as
Exhibit “L.”

9. Also, in compliance with the Rules, three (3) passport-size

colored photographs of petitioner, with her specimen signatures at the

bottom portion thereof, as well as impressions of her notarial seal are

attached hereto as Exhibits “M” and “N,” respectively.

10. Petitioner intends to apply for the renewal of her Commission

as Notary Public for and in the City of Caloocan, Metro Manila for a period
of two (2) years for the term ending on. Petitioner possesses all the
qualifications and none of the disqualifications for appointment as Notary
Public. Petitioner has never been convicted of any crime involving moral
turpitude or of any crime of whatever kind or nature. A copy of the
Clearance issued by the National Bureau of Investigation (“NBI”) in favor
of the petitioner is attached hereto as Exhibit “O.”

WHEREFORE, Petitioner respectfully prays that her application for

the renewal of her commission as Notary Public for and in the City of
Caloocan, Metro Manila for a term of two (2) years ending on be granted.
