CONSENT FORM

The Correlation of Screen Time using laptop and Academic Performance of Senior High

School Students of San Lorenzo Ruiz College of Ormoc

You are invited to participate in a research study about The Correlation of Screen

Time using laptop and Academic Performance of Senior High School Students of San Lorenzo

Ruiz College of Ormoc. The goal of this research study is to determine the correlation of screen

time using laptop and academic performance of the Senior High School Students.

This study is being conducted by Jemikko V. Baguion, Karl K. Nival, Lovelie Mae G.

Panilag, Leanne Imelie J. Poliquit, Ryan Rey R. Rosalijos and Airena Rain T. Tampos in

compliance with our Research Proposal.

Participation in this study is voluntary. If you agree to participate in this study, you

would be asked 10 different questions. The questions include the specific topic about the

research title which is answerable by yes or no.

Participating in this study will give you more ideas and will help you to manage your

screen time in using laptop and your academic performance.

The information you will share with us if you participate in this study will be kept

completely confidential.
