AFFIDAVIT OF GUARDIANSHIP

I, ERLINDA VILLAMOR SEMENIANO, Filipino, of legal age, married and a resident

of Purok 6, Brgy. 103, Palanog, Tacloban City, Philippines under oath declare:

1. That my granddaughter, CHRIS JANE SEMENIANO who is the child of my

daughter Jennifer V. Semeniano was born on 24 July 2016, her father is Christopher
Casner;

2. That at present, CHRIS JANE SEMENIANO is under my care and living with me
since she was (1) year old until now since her parents were separated;

3. That I am now applying for a certified true copy of the live birth certificate of CHRIS
JANE SEMENIANO with the Philippine Statistics Authority;

4. That by virtue of the foregoing circumstances, I stand as the de facto guardian of

CHRIS JANE SEMENIANO;

5. That this Affidavit is not contrary to law or morals and has no intention of fraud;

6. That I am executing this Affidavit of Guardianship to declare the truth of above-

narrated circumstances and for whatever legal purpose this may serve.
