I, RAUL ROSARIO MADERAZO, of legal age, married, Filipino, and a resident of

Tacloban City after having been sworn to in accordance with law, do hereby depose and say:

1. That I was born on 11 July 1963 to parents ANACORITA JEMENEZ ROSARIO

and JOVEN VERADOR MADERAZO at Tacloban City, Philippines;

2. That my birth was registered with the Office of the Civil Registrar, Tacloban,
however my name therein was written only as “ROSARIO MADERAZO”;

3. That in my other pertinent documents, my true and correct name is “RAUL

ROSARIO MADERAZO”;

4. That I am executing this affidavit to attest to the truth and veracity of all the
foregoing facts in support of my application to supply my First Name as “RAUL” to appear
as RAUL ROSARIO MADERAZO in my Certificate of Birth at the Office of the Civil
Registrar of Tacloban, and for whatever legal purpose this Affidavit may serve.
