AFFIDAVIT OF UNDERTAKING

I, LOLITA GERMANES HAMAC, Filipino, of legal age, and a resident of

Brgy. 6-A, Sto. Niño Extention, Tacloban City, under oath declare:

1. That I am now processing before the Business Permits and Licensing Office,
Tacloban City the renewal of the Business Permit of UNYX BOARDING
HOUSE located at Brgy. 6, Santo Niño Extension, Tacloban City;

2. That the said business is situated on a lot owned by the Government;

3. That if in any case the lot will be used by the government for public
purposes or any other purpose affecting my business situated thereon, in
whole or in part, I hereby undertake to voluntarily remove my business
establishment and waive any rights and claims against the City Government
of Tacloban and would not hold them liable if any damages/injury incurred
thereof;

4. That I am executing this Affidavit of Undertaking to declare the truth of

above-narrated circumstances and for whatever legal purpose this may
serve.
