AFFIDAVIT OF LOSS

KNOW ALL MEN BY THESE PRESENT:

I, ERYL ROYCE R. NAGTALON, Filipino, of legal age and presently residing

at16 Targa St. Village East, Cainta, Rizal, do hereby depose and state that:

1. I have a bank account with AUB (304-01-000596-5) and RCBC

(7590695045);

2. I have an ATM for both accounts;

3. The said my ATM Cards were misplaced and lost;

4. The whereabouts of my ATM Cards can no longer be ascertained despite

diligent search made and undertaken;

5. I am executing this affidavit to attest to the truth of the foregoing facts and to
request for a replacement of my ATM Cards.
