A supervisor was walking towards Tower E Lift
lobby in ground floor through the OTS area, a
Bangalore Godrej 24
paint roller/brush fell from the OTS infront of
him. No injury was reported.
It was reported that the vehicle moving out of
the site after unloading of cable drums. Its mirror
Bangalore Godrej 24 had accidentally came in contact with the site
gate due to which the gate fallen . The gate was
refixed. No injuries were reported.
It was reported that a workman Shifting pipe
pieces from 7th floor to store area got Slipped as
Bangalore Godrej 24 he was not wearing shoe properly but survived
the fall as he held the staircase hand rail. No
injuries reported.
Corrective/Preventive Action Recommended
Learning from Incident
1. During work permit approval physical inspection is
conducted.
Work area shall be maintained barricaded,
2. OTS area will be barricaded & work under process signage
Loose material/Painting roller or brush
will be displayed while carrying work inside OTS area shall not be kept on slab edges
3. Roller/brush will be tied to the harness to prevent
accidental fall.
1. Security team has been educated on handling traffic at
site
2. The gate was re fixed
3. Security personal will communicate the learnings of the Road safety precautions must be followed
incident during driver induction.
5. Signage has been displayed to create caution/awareness
