Incidents : Rain Water Tank Water Overflow to Ditch

Responsible Shift : 1st Shift
Members of Shift : Shandy ( Chief Operator ), Julian Rizaldi ( Inspector)
Current Shift : Night Shift
Incidents Description :

1. at 04:00 am, Shandy found that #1 MACT turbidity is higher than 5 ( the water is dirty )
2. Shandy asked Julian to slowly close rain water tank main outlet valve to #1 MACT and open
bigger rain water tank main outlet valve to Sludge Concentration Basin.
3. At that condition, 2 pumps rain water pumps have been put into automatic position.
4. When level is high, only #1 pump interlock start up, but #2 didn’t start up.
5. When Julian tried to start manually #2 pump, the water had overlowed to ditch.
6. And then Julian directly open again valve to #1 MACT,
7. After 1 minutes there was no overflow anymore.

Analisys Mistake:
1. After the water overflow, Shandy directly told to Captain ( 路 丽 鹏 ), unfortunately 路 丽 鹏
didn’t understand about the incidents.
