● I have been advised that I am free to withdraw my consent and discontinue participation
in the investigation at any time without prejudice.

● I confirm that the interview will last approximately 15-30 minutes.

● I have read and understood the explanation provided to me.

● I understand that such interviews and related materials will be kept entirely anonymous.
I have been given a copy of the consent form.

thought best for this investigation.

● I wish to review the notes, transcripts, or other data collected during the investigation
interview. I agree that my interview is audio-recorded.

● I have understood that my participation is voluntary and unpaid.

● I am free to withdraw from this participation at any time for any reason.

● I understand that I am free to contact any of the people involved in the investigation to
seek further clarification and information.

Participant’s Responsibilities and Permission

I voluntarily agree to participate in this interview, and I know of no reason I cannot
participate. I have read and understand the informed consent for participation in this subject.
If I participate, I may withdraw at any time without penalty. I agree to abide by the rules of
