Dear Police Major Coderis III:

I write this letter in behalf of SUSAN TUGAP MAIZ who is the registered
over of SUZUKI Multi-cab with particulars to wit:

Color :

The above-described vehicle is currently under the custody of your station

after its involvement in an accident on May 13 , 2016 in Matiao, Pantukan,
Davao de Oro, which also resulted to the filing of Criminal Case No. 846-
2020 (6768) against MARIO MAIZ before the Regional Trial Court, Branch
57, Mabini, Davao de Oro. On September 7, 2021, the Honorable Court
ordered the dismissal of the case and the release of the subject vehicle.

Our client, however, is complaining that some parts of the subject of the
vehicle, while the same is under your station’s custody, are missing, to wit:

1. Dashboard;
2. Two side mirrors;
3. Two chairs;
4. Horn cover;
5. Seatbelt;
6. Oil engine cover;
7. Gear shift knob;
8. Brake hydronox;
9. Signal light lever;
10. Wiper lever;
11. Headlight lever;
12. Horn;
13. Header

By reason of the foregoing, our client is demanding for the immediate

replacement of the above-mentioned items. Hoping for your prompt action
on this matter.
