JOINT AFFIDAVIT
(of two disinterested persons)

We, NESTOR B. TRANGIA and JOAQUIN M. GARCIA, both Filipinos, both married, of
legal ages, and both residents of Brgy. Macabug, Ormoc City, Leyte, after having been duly sworn
to in accordance with the law, do hereby depose and say under oath THAT:

1. That we both personally know OSCAR C. NICODINO, a resident of, Brgy. Macabug,
Ormoc City, Leyte, being neighbors and close acquaintances;

2. That his Social Security System (SSS) records apparently reflect an error as to his
middle initial and birth year. His name as per SSS records is “OSCAR A. NICODINO”
and the birth year per SSS records is “1957”;

3. His real name, however, is “OSCAR CABILLO NICODINO” and the birth year is
“1956” as reflected in his Certificate of Live Birth;

4. We execute this affidavit to attest to the fact that “OSCAR A. NICODINO” and
“OSCAR CABILLO NICODINO” refer to one and the same person, and that his birth
year is “1956”, and for whatever legal purpose this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 13th day of September
2022 in Ormoc City, Province of Leyte, Philippines.
