I, ADELFA RIPDOS OUANO, a Filipino citizen and a resident of Lot 9

Phase 3 Sagkahan, Holy Family Village, Kananga, Leyte Philippines, after having
been duly sworn to in accordance with law, hereby depose and say, THAT:

1. That I am presently unemployed and neither am I engaged in any kind

of business;

2. That I am executing this affidavit for purposes of filing a petition for

correction of clerical error in my certificate of live birth;

3. That I am executing this affidavit to attest to the veracity of the

foregoing facts and circumstances and for whatever lawful purpose this may
serve.
