DOCUMENTARY REQUIREMENTS FOR BUILDING PERMIT APPLICATION
(Single Dwelling Residential / Commercial / Industrial / Institutional)

Five (5) sets filled up Application Forms (signed and sealed over the printed names of the duly licensed and registered professionals.

Five (5) sets of lot survey plan (with recent geodetic/relocation survey conducted by a Registered Geodetic Engineer).
Five (5) sets of design plans (building, structural, architectural, electrical, plumbing, mechanical, sanitary, electronics plans) signed and sealed over the printed names
of the duly licensed and registered professional.
Five (5) sets of design specifications.
Five (5) sets of bill of materials.
Developer's Clearance
Structural Design Analysis for all buildings/structures except one (1) - storey and single detached building/structure with total floor area of 20.00 sq. meters or less.
Electrical Design Analysis (signed & sealed by Professional Electrical Engineer).
Soil Boring Test for buildings/structures three (3) storey & higher.
Photocopy of the PRC I.D. & PTR of professionals in-charge of the design plans and work supervision.
Two (2) Certified True Copy of OCT/TCT, on file with the Registry of Deeds.
Two (2) Certified True Copy of Tax Declaration of land & existing buildings/improvements [land classification should conform with proposed use of the building/structure].
Two (2) copies of Tax Clearance (Original Copy).
Two (2) copies of Current Real Property Tax Receipt.
Duly notarized copy of the Contract of Lease, Contract to Sell or Deed of Absolute Sale in case the applicant is not the registered owner of the lot.
Environmental Compliance Certificate [for ECC covered projects].
Sangguniang Bayan Franchise for Private Markets (for applicable market projects).
NHA Clearance (for constructions within NHA area of jurisdiction).
Technical Description (NHA)
Traffic Impact Analysis for traffic generating buildings such as shopping malls or similar facilities that have very high volume of pedestrian and vehicular traffic.
Department of Health (DOH) clearance for construction of cellular site tower.
Mayor's Permit of Contractor.
Secretary's Certificate (for Corporation).
Filled up Application Form for Locational Clearance.

l All the fully accomplished forms and requirements/documents must be placed in a long plactic envelop except for the drawing plans.
l Only a COMPLETE and COMPLIANT APPLICATION WILL BE ACCEPTED.
l Bring the checklist with detachable Claim Stub when submitting your application.
l Keep your CLAIM STUB at all times, "NO CLAIM STUB, NO RELEASE'.
l Application is VALID until SIX (6) MONTHS. If after six (6) months the owner did not come back, said application will be CANCELLED and the submitted
plans and documents shall be DISPOSED.
