南方医科大学来华留学生入境返校承诺书

本人自觉遵守中国关于新冠肺炎疫情防控的法律法规，自觉
遵守广东省新冠肺炎疫情防控各项管理规定，按照对自己负责、
对他人负责、对学校负责的原则，承担疫情防控社会责任，郑重
作出如下承诺:
一、本人已完成新冠疫苗全程接种；
二、入境前 14 天，如出现发热、干咳等症状及时就医，并报
告辅导员老师；
三、入境前按照中国大使馆最新要求完成相应核酸检测；
四、入境后按照所在地疫情防控要求完成隔离医学观察、居
家健康监测、集中健康监测与核酸检测，并自愿承担相应费用；
五、入境返校途中,严格做好个人防护，避免与外人接触；
六、返校后，持续关注自己的健康状况，戴口罩、常通风、
勤洗手，减少聚集，按时参加学校组织的核酸检测，如有身体不
适，及时就医，并报告辅导员老师；
七、保证遵守学校及卫生健康部门新冠肺炎疫情防控有关规
定；
八、保证遵守学校各项规章制度。
本人已知晓上述事项,因违反规定产生的一切后果和责任由我
本人承担。

 Letter of Commitment

I voluntarily abide by China's laws and regulations on the prevention and control
of COVID-19, and abide by the regulations of Guangdong Province on the
prevention and control of COVID-19. In accordance with the principles of being
responsible for myself, others and the university, I undertake social
responsibilities for the prevention and control of COVID-19, and solemnly make
the following commitments:
1. I have been fully vaccinated against COVID-19.
2. 14 days before Entering China, if I have symptoms such as fever or cough, I
will seek for medical attention in time, and report to the supervisor.
3. Before Entering China, I will complete the corresponding nucleic acid tests
according to the latest requirements of the Chinese embassy.
4. After entering China, I will complete the centralized quarantine, home
health monitoring, centralized health monitoring and nucleic acid tests in
accordance with the local epidemic prevention and control requirements, and
will voluntarily bear the corresponding expenses.
5. On the way back to school, I will strictly take personal protection and avoid
contact with outsiders.
6. After returning to school, I will continue to pay attention to my health, wear
masks, ventilate frequently, wash hands, reduce aggregation and participate in
the nucleic acid tests organized by the university. If I have symptoms such as
fever or cough, I will seek medical treatment in time and report it to the
supervisor.
7. I promise to abide by the relevant regulations of the university and the
health authorities on COVID-19 prevention and control.
8. I promise to abide by the rules and regulations of the university.
I have been aware of the above matters. If I violate the regulations, I shall bear
all the consequences and responsibilities.
