Form?

1. Patients come with cards, medical records books, letters, refferels

2. Triage is performed
3. Out-patient record filled by diagnose nurse
4. Audiometrist(only done when dignosed nurse when reffered)
5. Review can be done by using form 4 until patient improves. If not patient is sent to surgeon
6. Admission form and consent operation process, it depends on operation.
7. Admission is dependent on severity.
8. Discharge if successful recorded inside card
9. Review recorded inside card
10. For throat and nose general admission process
11. Just record in their cards their findings
12. Reports
a. ICD coding
b. Data taken from daily ENT clinic, out-patient recording
c. Monthly reporting on operation and ENT out patient recording
