Attached is a supplement to the Zillow Rental application related to the unit for which you are applying. A
separate application will need to be completed for non-related individuals wishing to be listed as co-tenants.
Once your application has been received it generally takes two (2) to five (5) business days for us to process
your application, contact references and verify information on your application.
Please make sure you provide the following information with your application:

Note: Incomplete applications may be disqualified. There are two application steps, first the
Zillow application accessed via their website, and our enclosed application. Please fill out this
form as completely as possible - if the field is pre-marked "NA" you can ignore it as the
information will be provided via the Zillow applicaiton. There may be some redundancy
between the two applications, in which case supply the information on our application.
To hold the unit, include a minimum deposit of one month’s rent with your application. Including
the deposit with your application temporarily removes the unit from the market pending approval of
your application and signing of the lease. If your application is rejected your deposit will be
returned to you in full within ten (10) business days. Upon approval of your application and lease
presentation you will have five (5) calendar days to sign the lease and pay the remaining security
deposit. If upon approval of your application you decline to sign the lease you forfeit your deposit.

Completed applications may be sent via email, fax, US Mail or delivered in person. If the application is
emailed or faxed please mail/deliver/venmo applicable fees and deposit. Consideration for emailed or faxed
applications can not be completed until the application fee has been received.

This is a legal document which imposes legal obligations on you and may under some
circumstances result in forfeiture of your earnest money. PLEASE READ CAREFULLY.
APPLICANT hereby authorizes LESSOR to verify accuracy of all statements in in this application, and also authorizes all employ-
ers, previous landlords, mortgage holders, all banks and any other creditor listed on the lease application, to release all information
concerning me or my account(s), my rental history or payments, my employment history, including the likelihood of future employ-
ment - for the purpose of verifying this application, and determining my ability to afford the contractual obligations of the LEASE.
This authorization shall be continuing during the term of the LEASE or any renewal of the LEASE; to update or otherwise verify
new information including, but not limited to LESSOR’S rights to inquire from time to time, from banking institutions, whether
there are sufficient funds to pay check, or checks delivery by LESSEE or LESSOR.
I release all such persons delivering said and all other information, from any inadvertent error, whether communicated either orally,
or in writing or over the telephone.
APPLICANT(S) certifies and represents that all information on this APPLICATION is correct, that he/she/they are not breaking
his/her/their lease, that he/she/they have paid their rent/mortgage payments in a timely fashion for the past twelve (12) month period,
that they have received no notices of Lease Termination or Eviction; and that they have not filed for bankruptcy within the past five
(5) years, are presently financially solvent; that no adverse credit information is on record - except as follows:

Upon approval APPLICANT agrees to sign LEASE and pay Security Deposit (1 1/2 months rent) plus first
months rent, within five days of being notified of approval by LESSOR.
By signing my signature to this application, I hereby verify that I am the person whom I represent myself to
be, and all the above information is true and accurate. Inaccurate or false statements will be cause for immedi-
ate rejection of the application and forfeiture of the application fee.
