AFFIDAVIT OF LOSS

I, RACHEL BALICAG COBCOBO, of legal age, single, Filipino citizen, and a

resident of No. 121 Lower Quirino Hill, Baguio City, after having been duly sworn to in
accordance with law, do hereby depose and say to wit:

1. I was issued a Tax Identification Number 924-594-829-000 (a.k.a. TIN ID) by the
Bureau of Internal Revenue (a.k.a. BIR) of Baguio City;

2. Unfortunately, my wallet wherein the said was placed got lost when I was on my
way home last Saturday, 24 November 2007;

3. I exerted diligent efforts to locate the same but all to no avail;

4. The said loss was neither due to its confiscation nor for any reason of such nature;

5. I am executing this affidavit to attest to the truth of the foregoing facts for all legal
intents and purposes, in particular, to request the BIR of the City of Baguio to
issue me a new Tax Identification Number in lieu of the lost one, after payment of
the required fees thereof.
