Chapter 1
THE PROBLEM
Introduction
Cerebrovascular accident (CVA) or also referred to stroke. CVA is one of
the leading cause of death and a leading cause of long-term disability to many
Filipinos. Usually, stroke happens unpredictably due to risk factors including
stress level, diet, exercises, lifestyle, and/or may have past health history of
comorbidities leading to cerebrovascular accident or stroke. On the other hand,
post-stroke patients who were undergoing rehabilitative care at home still need to
comply with the medical attendance in both public health services and hospital
services to support them and help to prevent the risk for reoccurrence of severe
case of stroke that may lead to death.
Further, risk management and health prioritization in triage area of
emergency department should be accurate and concise, especially when dealing
patient medical history such like patient with history of stroke. Triage process
may vary on how triage officers extract health information from the patient,
particularly when subjecting a post-stroke patient in an interview. Hence, triaging
of post-stroke patient may be difficult, especially when extracting subjective data

and past health history from the patient. Therefore, consistency of assessment
data vary on the next process affecting the quality of management and treatment
to be given to post-stroke patient.
The lack of medical attendance of post-stroke patients is secondary to the
disintegration of the once integrated referral system involving both public health
services and hospital services, where triage officers in emergency department
were challenged in conducting a thorough health assessment. This challenges
reflect to the numerous number of population who had stroke. It has a young
population with 90 percent in the age group below 55 years old. Only 10 percent
are in the age group above 55 years old. Stroke is considered the third leading
cause of deaths with a crude death rate of 58.1 per 100,000, next to the diseases
of the heart and malignant neoplasms, among Filipinos. In the latest available
Philippine Health Statistics (2016), it was reported that more than half 54 percent
of those who have died from stroke failed to seek medical attention.
In other countries including USA, European Countries, Asian Countries,
and in Australia, a triage scale that will ensure the completeness of the data of
the patient (Yousif et al., 2020). Also, there are triage scale created and used in

the said countries that will help in triaging of stroke patients and even post-stroke
patients. Hence, triage scale for post-stroke patients helps to review the ongoing
monitoring process and outcomes which will help to increase the efficiency of
care (Rudd et al., 2021).
Recordings in triage area in Emergency Department is very important in
order to address the needs of the patient and help to attain the quality of
management and treatment. So, the researcher would like to examine triage
process in handling post stroke patients in Camarines Norte Provincial Hospital.
The researcher believe that Camarines Norte Provincial Hospital still lacks its
quality in handling triage process. On the other hand, Camarines Norte Provincial
Hospital is known as the worst hospital in the Philippines. In this regard,
examining the process in handling post stroke patients in Camarines Norte
Provincial Hospital will serve as the basis for innovation to ensure the quality or
standards of health care services, particularly on how triage process takes place.
Furthermore, this study would like to proposed triage scale for post-stroke
patients which will serve as a tool to be used during triage process.

Statement of the Problem
This study aims to proposed a triage scale for post stroke patients which
will serve as a basis for improving nurses’ prioritization. Specifically, it will seek
answer on the following sub-questions:
1. What observations can be noted about the triage process in
Camarines Norte Provincial Hospital for post-stroke patients in terms of:
A. Triage Form, and;
B. Hospital Policy for Triage Process?
2. What triage scale for post-stroke patients can be proposed to improve
case prioritization?
3. How effective is the proposed triage scale for post-stroke patients in
the triage process in Camarines Norte Provincial Hospital?
4. What recommendations can be proposed to strengthen the utilization
of triage scale for post-stroke patients to enhance triage case prioritization in
Camarines Norte Provincial Hospital?

Assumptions of the Study
The following are the assumption of this study:
1. There are distinct observations about the triage process in Camarines
Norte Provincial Hospital for post-stroke patients in terms of triage form and
hospital policy for triage process.
2. There is a triage scale for post-stroke patients can be proposed to
improve case prioritization.
3. There are difference opinion about the effectiveness of proposed triage
scale for post-stroke patients in the triage process in Camarines Norte Provincial
Hospital.
4. There are different recommendations that will strengthen the utilization
of triage scale for post-stroke patients to enhance triage case prioritization in
Camarines Norte Provincial Hospital.

This researcher will aim to proposed a triage scale for post-stroke patient
which will serve as a basis for improving nurses’ prioritization. The scope of this
study is to examine the triage process in Camarines Norte Provincial Hospital for
post-stroke patient; create a draft of triage scale for post-stroke patients to
improve case prioritization; examine the effectiveness of triage scale for post-
stroke patients in the triage process in Camarines Norte Provincial Hospital; and
the proposed recommendations that will strengthen the utilization of triage scale
for post-stroke patients to enhance triage case prioritization in Camarines Norte
Provincial Hospital.
This study will use a questionnaire and a rubrics in order to gather data
regarding the proposal of triage scale for post-stroke patients. This study will
involve clinical chief nurse in Camarines Norte Provincial Hospital who will
provide data about the triage process background of Camarines Norte Provincial
Hospital and the twenty-four (24) ER Nurses and Nursing Attendants who will
evaluate the effectiveness of the proposed triage scale for post-stroke patients.
The data will be analyzed using frequency, percentage, and mean.

Significance of the Study
The findings of the study will focus in creating triage scale for post-stroke
patients to improve nurses’ prioritization. Similarly, in deciding to conduct this
study, the researcher believes that the results will benefit the sector and persons
as follows:
ER Nurses and Triage Team. The results of the study will enrich the
triage process by using triage scale for post-stroke patients. Also, this study will
equipped ER nurses and triage team in creating a full-scale assessment among
post-stroke patient who wants to consult and confined in Emergency Department.
Moreover, this study will reduce the workload and reduce the time spent in
creating a triage data among post-stroke patients.
Other Health Care Workers and Health Care Professionals. The
results will provide a complete triage data which will benefit the entire health care
workers and health care professionals, especially to the persons involve in the
care of post-stroke patients. Also, this study will help to reduce the time and

workload in asking about the history of attacks of post-stroke patients by referring
on the data in the proposed triage scale made. Moreover, this will also help to
move forward from assessment by creating an accurate diagnosis with the use of
data covered in the proposed triage scale for post-stroke patient.
Emergency Department. From the results of the study, the researchers
envisioned the needs of the emergency department on the materials and tools
that will help to lessen the time In creating an assessment data. Also, the
proposed triage scale for post-stroke patient will help to improve the triage
process employed in emergency department and help to save time and effort by
offering a direct and close-ended questions that will help to reduce the risk for
injury among post-stroke patient who enters in triage area.
Camarines Norte Provincial Hospital. The results will benefit the
standards and quality of triage process in Camarines Norte Provincial Hospital
with the use of triage scale for post-stroke patient. Also, this study will also
require their approval on how triage scale will help to reduce the time and effort
in creating a full-scale assessment among post-stroke patients.
Student Nurses. The result of this study will provide them with
information about the essence of triage process. This will also cognize them as to
the vital information that are needed when creating a triage data among post-
stroke patients at Camarines Norte Provincial Hospital, where they affiliate as

part of their Related Learning Experience (RLE). More so, this will serve as an
insight that will encourages them to create triage scale that will benefit the triage
process, particularly creating assessment data.
Clinical Instructors. The result of this study will give them empirical basis
to strengthen clinical teaching-learning activities. Furthermore, they could
formulate measures to help student nurses in developing their skills in creating
an assessment data when a post-stroke patient is in triage area. In that way,
student nurses will be able to prepare them in the future undertake of their
profession.
Future Researchers. This study will give knowledge and serve as their
reference material that may help them on their future study. The future
researcher/s can do some extension of this study. Moreover, this study will serve
as a blue print in resolving gaps and conflicts over time in terms of research
breakthrough.

Effectiveness. It refers to the degree which something attained a success
in producing a desired results. In this study, it examines the effectiveness
proposed triage scale for post-stroke patients in promoting case prioritization.
ER Nurses. It refers to health care workers assigned in Emergency
Department of Camarines Norte Provincial Hospital. In this study, they will serve
as the study participants which will evaluate the effectiveness of proposed triage
scale for post-stroke patients.
Observations. It refers to the action or process of observing something or
someone carefully in order to gain information. In this study, the observations will
be noted from the Clinical Chief Nurse of Camarines Norte Provincial Hospital in
regards the triage forms, and the policy for triage process employed in
Emergency Department.
Post-Stroke Patients. It refers to the patient with occurring in or being in
the period following a stroke. In this study, the proposed triage scale focuses
among post-stroke patients.
Recommendations. It refers to a suggestion or proposal as to the best
course of action, especially one put forward by an authoritative body. In this
study, it will serve as the outcome of the study in order to continue to widen the
scope of the study, by not just proposing a triage scale for post-stroke patients by

also creating a recommendations to increase its recognition to widely used it in
Emergency Department.
Triage Area. It refers to place where patients who desired for consultation
and admission will be initially taken their data in this area before they will be
transferred in Emergency Room.
Triage Form. It refers to the tool used by the triage team when extracting
data from the patients, especially post-stroke patients.
Triage Scale for Post-Stroke Patients. It refers to the researcher-made
triage scale for post-stroke patients, where in subjective and objective data will
be taken in order to create a complete and accurate data before it can be
referred to consulting physician in Emergency Department.
Triage Team. It refers to individual/group that creates an initial data
before the patient able to seek consultation from the physician in Emergency
Department.
