ASSESSING THE SKIN, HAIR AND NAILS

INSTRUCTION TO PRECEPTORS :
Rate the student by writing the number that corresponds to the equivalent skill performed for
each step.

1. The assessment is completed without the
patient experiencing anxiety or discomfort,
the findings are documented, and the
appropriate referral is made to the other
healthcare professionals, as needed, for
further evaluation.
PREPARATION
1. Introduce yourself, and verify the client’s identity.
Explain to the client what you are going to do,
why it is necessary, and how the client can
cooperate.
2. Perform hand hygiene; observe other appropriate
infection control procedures.
3. Provide for client privacy.
4. Inquire if client has any history of the following:
 Pain or itching
 Presence and spread of any lesions, bruises,
abrasions, or pigmented spots
 Skin problems
 Associated clinical signs
 Use of medications, lotions or home remedies
 Excessively dry or moist feel to the skin
 Tendency to bruise easily
ASSESSMENT
1. Inspect skin color
2. Inspect uniformity of the skin color
3. Assess edema, if present.
4. Observe and palpate skin moisture.
5. Palpate skin temperature.
6. Note skin turgor by lifting and pinching the skin on
the extremity
7. Document findings in the clients record.
ASSESSING THE HAIR
1. Inquire if the client has any history of the
following:
 Recent use of hair dyes, rinses, or curling or
straightening preparations.
 Recent chemotherapy
 Presence of disease
2. Inspect the evenness of growth over the scalp
3. Inspect hair thickness or thinness.
4. Inspect hair texture and oiliness
5. Note presence of infections or infestations by
parting the hair in several areas and checking
behind the ears and along the hairline of the neck.
6. Inspect the amount of body hair.
7. Document findings to the clients record.
ASSESSING THE NAILS
1. The assessment is completed ithout the patient
experiencing anxiety or discomfort, the findings
are documented, and the appropriate referral is
made to other healthcare professionals, as
needed, for further evaluation.
PREPARATION
2. Inquire if client has any history of the following:
 Diabetes mellitus
 Peripheral circulatory disease
 Previous injury
 Severe illness
ASSESSMENT
3. Inspect fingernail plate shape to determine its
curvature and angle.
4. Inspect fingernail texture and bed color
5. Inspect tissue surrounding nails
6. Perform blanch test of capillary refill. Press two or
more nail between your thumb and index finger,
look for blanching and return of pink color to nail
bed to check for adequate oxygenation
7. Documents finding in clients record.
