AFFIDAVIT OF LOSS

We, SPOUSES MELCHOR C. CASTILLON and DIVINA R. CASTILLON,

both of legal age, Filipinos, and residents of Brgy. Cabagiao, Calinog, Iloilo, after having
been sworn under oath, state that:

1. Last September 27, 2011, we purchased through an auction by the Office of

the Treasurer of the Municipality of Calinog a property covered by Lot 4048 and Transfer
Certificate of Title No. T-16806 situated in Barangay Cabagiao, Calinog, Iloilo, registered
under the name of Eulalio C. Casa;

2. In the said auction, we received the Owner’s Duplicate Copy of Transfer

Certificate of Title No. T-16806 and became the holders thereof;

3. When we tried to process the transfer of the title into our names sometime in
2019, the owner’s duplicate title of the said property can no longer be found;

4. Despite diligent efforts exerted, we could no longer locate, find, or recover the
owner’s duplicate title of the said property, and to the best of our knowledge, the same is
truly lost;

5. We execute this affidavit to attest to the truth of the foregoing statements.
