JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

We, VERONICA B. JIMENEZ and MARLYN D. LIMOS, both of legal age, Filipino
citizens and residents of San Jacinto, Pangasinan and Brgy. Buenlag, Mangaldan, Pangasinan, under oath
do hereby depose and state:

1. That we personally know JEYAN GALILEE BUGAYONG QUIDILIG who was born
on February 01, 2018 at Region 1 Medical Center (Annex) Dagupan City, Pangasinan to the parents
MANUEL JUNI QUIDILIG JR. and JENELLE PARAJAS BUGAYONG, because we have been
closely acquainted with the family for a very long time;

2. That JEYAN GALILEE BUGAYONG QUIDILIG and her MANUEL JUNI

QUIDILIG JR. and JENELLE PARAJAS BUGAYONG, have been residing in Brgy. Alitaya,
Mangaldan, Pangasinan for a long time now;

3. That due to the negligence and/or inadvertence on the part of the parents of JEYAN
GALILEE BUGAYONG QUIDILIG, her birth was not reported and registered immediately with the
Office of the City Civil Registrar of Dagupan City, Pangasinan;

4. That we hereby attest and declare and attest that JEYAN GALILEE BUGAYONG
QUIDILIG is the child of MANUEL JUNI QUIDILIG JR. and JENELLE PARAJAS
BUGAYONG;

5. That we are executing this Joint Affidavit to attest to the truth of the foregoing and for
whatever legal purpose it may serve;
