ORGANIZATIONS (HMOs)

SUBJECT : SURVEY RELATIVE TO THE COVID-19 PANDEMIC

In order for the Insurance Commission to continuously monitor the impact of the
COVID-19 pandemic on our regulated entities, all life and non-life insurance
companies, mutual benefit associations and health maintenance organizations are
hereby directed to participate in the "Survey on COVID 19" at
https://bit. ly/I CSurveyOnCOVI D1 9 or https://forms.gle/dX7YFkLX8Fmq2g8B9 to be
submitted on or before 20 April 2022 (covering claims paid from 01 January to 31
March 2022).

All information to be provided shall be treated with utmost confidentiality. The

Insurance Commission strictly follows the general principles and rules on Data Privacy
protection.
