AFFIDAVIT OF UNDERTAKING

I, __________________, Filipino and with postal address at __________________, after having been
sworn with the law do hereby depose and say:

1. That I am bonafide student of the City of Malabon University taking up the program
__________________ under the college of __________________

2. That as such, I am a city scholar and should have been an excellent corresponding to the program that
I am taking the academic standards that this college observes.

3. That I should maintain a GWA of 2.00 (for board – based programs) or 2.75 (for non – board – based
programs) so that I can be considered as a student with good standing.

4. Last semester, I failed to maintain the required GWA above stated, the reason why my status has
been designated with warning.

5. That the college reconsidered me for the last time provided I undertake that from now on, I will
maintain GWA per semester for the rest of my stay here in CMU.

6. Further, I should maintain the prescribed grades in all of my subjects that I am currently enrolled.

7. That if ever I will fail to meet the required GWA as above stated, I shall voluntarily file for honorable
dismissal.

8. Neither shall I seek any further reconsideration nor seek any help or assistance to mediate of my
behalf from any person in the city hall or from other offices, either government or private;

9. That I execute this affidavit of undertaking together with and under strict guidance of my guardian.
