Submit your Application Folders to the Office of the Recruitment and Selection
Section, BFP National Headquarters, Agham Road, Quezon City, NCR not later than
May 6, 2015.

The folder shall contain the following:

a. Application Letter indicating the position and item applying for;
b. One (1) Handwritten Personal Data Sheet (PDS) usng black ballpen only. No sign
pen;
c. One (1) Photocopy of Authenticated Transcript of Records and College Diploma;
d. One (1) Photocopy of Authenticated Certificate of Eligibility (CSC/PRC);
e. One (1) Photocopy of Certificate of Live Birth issued by the National Statistics
Office;
f. One (1) Photocopy of Certificate of Marriage issued by the National Statistics
Office (for married female applicants);
g. One (1) Photocopy of NBI Clearance: and
h. Photocopy of Certificates of Training (if any)

 Presidential Decree No. 907 (Honor Graduates - Civil Service Commission);

Commission);
 Officer-in-Charge Navigational Watch / Engineering Watch (Marina)

Documentary Requirements

Applicants must submit the following documentary requirements in a properly-tabbed

folder to the Human Resource Management Division of BFP National Headquarters
or any BFP Regional Offices near you:
 Application Letter;
 2 handwritten / computerized Personal Data Sheet (PDS);
 Authenticated copy of College Diploma;
 Authenticated copy of Transcript of Record;
 Authenticated copy of Second Level Eligibility;
 PSA Certificate of Live Birth;
 PSA Certificate of Marriage Contract (Female only);
 Brgy, Mayor, Police, NBI, MTC and RTC Clearances;
 Certificate of Waiver if applicable
Waivers (any of the following)
 National Certificate for Indigenous Peoples
 National Certificate for Muslim Filipinos
 Professional Drivers License with restriction code 1, 2 & 3
 Certificate of Fire Volunteer for atleast 6 months
