    REQUIREMENTS FOR MATERNITY LEAVE REQUIREMENTS FOR TRAVEL ABROAD (3 copies each)

    (3 copies each)
    - Application for leave (Form 6) - Form 6 (with director’s name)
    - School District Clearance - Clearance from School Head
    - First Appointment - Endorsement from School Head
    - Medical Certificate duly signed by Physician - Certification (who will handle his/her classes)
    *Pls. indicate employee no. and basic salary - Letter of Intent
    - Division Clearance
    REQUIREMENTS FOR SICK LEAVE (6 days & above) REQUIREMENTS FOR STUDY LEAVE
    (3 copies each) (3 copies each)
    - Attached service credits (if any) - Study leave grant under the Magna Carta for Teachers (RA 4670)
    - Application for leave (Form 6) - Form 6
    - Medical certificate duly signed by Physician - Service Record
    - District/School Clearance - Schedule of Study leave
    - Teacher 1 appointment - Performance Rating
    *Pls. indicate employee number and basic salary - Certification that the applicant is more than seven (7) years in service
    - Certification (who will handle his/her classes / no substitute allowed)
    - Money and property Clearance
    - Administrative Clearance
    - District Ranking for sabbatical leave
    REQUIREMENTS FOR VACATION LEAVE (10 days & above) REINSTATEMENT REQUIREMENTS FROM MATERNITY LEAVE
    (3 copies each) (3 copies each)
    - Application Letter for Vacation Leave - Application letter
    - Application for leave (Form 6) - Endorsement from School District
    - School/District Clearance - Birth Certificate of the child
    - Medical Certificate duly signed by Physician
    *Pls indicate employee number and basic salary
    REQUIREMENTS FOR PATERNITY LEAVE REINSTATEMENT REQUIRMENTS FROM SICK LEAVE FOR 10 DAYS & ABOVE
