July 4,  Information When we arrived at the Rural Health Unit (RHU), the staffs accommodate and

afternoon. And I assist her in sorting the Patient Philhealth (Beneficiaries)
documents/files/forms and sorting the RTH (Return To Hospital) claims.
July 5,  Information I arrived around 7:30 am. While waiting for the RHU Clerk on her arrival in
2022  Office of the RHU since I am assigned to assist her starting today. I noticed that there are
Municipal so many patient and they’re almost student concern for medical certification
Health Officer as one of the requirements for enrollment. So, I help/assist more or less 10
students to get their height and weight after they filled out the form. I also
learned to get the Blood Pressure of the patient. I went to the office of the
assigned doctor for the signature. After that the RHU Clerk arrived and I
followed her entering the Office of the Municipal Health Officer to continue
assisting her in Sorting the Patient Philhealth (Beneficiaries)
documents/files/forms and sorting the RTH (Return To Hospital) claims.
Later, in the afternoon she taught and guide me encoding the patient
Philhealth (Beneficiaries) Information and consultation record to be
submitted in the iclinicsys ( Electronic Medical Record).
July 6,  Office of the Continue encoding the patient Philhealth (Beneficiaries) Information and
2022 Municipal consultation record to be submitted in the iclinicsys ( Electronic Medical
Health Officer Record). That was my work from morning to afternoon.

July 7,  Information Assisting incoming 1st year college students for medical certification before
2022  Office of the the assigned doctor attend to the meeting/seminar and encoding the patient
Municipal Philhealth (Beneficiaries) Information and consultation record to be
Health Officer submitted in the iclinicsys ( Electronic Medical Record). And later in the
afternoon the RHU Clerk also went to that Seminar/Meeting but my task is to
continue encoding even if she’s not in the office.
July 8,  Office of the Continue encoding the patient Philhealth (Beneficiaries) Information and
2022 Municipal consultation record to be submitted in the iclinicsys ( Electronic Medical
Health Officer Record). That was my work from morning to afternoon.
