DLSZ PA GENERAL ASSEMBLY

I have read the letter and notice of VIRTUAL GENERAL ASSEMBLY AND ANNUAL
□ MEETING (“General Assembly”) of the DLSZ (De La Salle Santiago Zobel School)
Parents’ Association, Inc. (the “Parents’ Association”) to be held on August 27, 2021,
Friday, from 8:00am onwards.

□ We will attend (both parents or guardian(s)).

□ One of us will attend (___Father ___Mother ___Guardian).

□ Sorry, we will not attend the General Assembly and Election.

Please see the PROXY FORM below and accomplish as necessary. Under Article II, Section 1 of
the Bylaws of the Parents’ Association, a family, regardless of number of attendees, is
considered one member, and a member is entitled to one vote per child enrolled in De La Salle
Santiago Zobel School (“DLSZ”).

The undersigned, a member of the Parents’ Association, hereby makes, constitutes and appoints
(please check only one).

□ Ms. Shalimar B. Reyes, Chairman, Parents’ Association

□ My own representative who is also a member of the Parents’ Association, whose

name is ___________________________________, as my true and lawful proxy, for me
and in my name, to be counted and be included as if present at the Parents’ Association
Virtual Annual General Assembly to be held on August 27, 2021, Friday, for purposes of
determining the existence of a quorum, and for the same purpose at any adjournments or
postponements thereof. I am fully aware that under Article VI, Section 10, proxies shall
be valid in all meetings for quorum purposes only, and votes of proxies will not be
counted for purposes of voting (i) on any issues that will be raised at the General
Assembly, and (ii) in the elections.
