Summary: The exploratory laparotomy is a laparotomy performed to obtain information not

available through clinical diagnostic methods. It is usually performed on patients who have
acute or unexplained abdominal pain, on patients who have had abdominal trauma, and on
patients who have a malignancy. Once the underlying pathology has been identified, an
exploratory laparotomy may be performed as a therapeutic procedure; it may also be used to
confirm a diagnosis in some cases (as in the case of laparotomy and biopsy for intra-
abdominal masses that are considered inoperable). These applications differ from a
laparotomy for a specific treatment, in which the surgeon plans and performs a therapeutic
procedure. The indications for and scope of exploratory laparotomy have shrunk over time as
sophisticated imaging modalities and other investigative techniques have become more
widely available. The growing availability of laparoscopy as a minimally invasive method of
inspecting the abdomen has reduced the use of exploratory laparotomy even further.
Nonetheless, the value of exploratory laparotomy as a quick and low-cost method of treating
acute abdominal conditions and trauma cannot be overstated.
