PARENTAL CONSENT

I hereby give my consent to my child Lizel Ermei G. Lambino to participate in the First City Providential
College (FCPC) Swimming Class and I hereby execute this agreement, waiver and release on his/her behalf.
I hereby state that my above-named child is physically able and capable to participate in said activity.
I also agree to assume all liabilities for my child/children may incur while at Los Arcos de Hermano Resort
for Swimming Class.
The First City Providential College and Los Arcos de Hermano Resort are not responsible for the loss of; or
any damage of my child's personal belongings.
I have carefully read this agreement, waiver and quit claim and fully understand its content. I am aware that
this is a release of liability and contract between myself, FCPC and the resort.
