Sesi 3b - Konsep Penyebab Penyakit 2011
PENYAKIT
What is a causal?
• “something that brings about an effect”
Webter’s New Collegiate
• Modern Epidemiology:
“an event, condition or characteristic that preceded a disease without which the
disease event either would not have occurred at all or would not have occurred until
some later time”
Rothman & Greenland, 1998
Konsep
• Ide atau rancangan yang diabstrakan dari
peristiwa yang kongkret
• Rancangan
• Gambaran mental dari objek, proses, atau
apa saja yang ada di luar bahasa, yang
digunakan oleh akal budi untuk memahami
hal-hal lain.
Definisi Penyebab
• 1. “yang menghasilkan efek” (Oxford English
Dictionary)
• 2. “sesuatu yg menimbulkan efek atau hasil
(Webster’s New Collegiate, 2nd ed)
• 3. “… suatu obyek, diikuti yg lain, dan setiap
obyek yg sama dng obyek pertama diikuti oleh
obyek yg sama dng yg kedua. Dng kata lain, bila
obyek pertama tidak terjadi, obyek kedua tidak
akan terjadi.” (David Hume)
Definisi Penyebab (lanj.)
• 🡪faktor penyebab adl suatu event, tindakan,
kondisi atau status alamiah yg baik sendiri
atau bersama-sama, memulai atau
memungkinkan event lain yang
menimbulkan akibat” (Rothman)
• “ suatu event, kondisi atau ciri yg
mendahului kejadian penyakit, yg tanpa hal
tsb penyakit tak akan pernah terjadi atau
terjadi at a later time”
Teori-teori penyebab penyakit
• Teori supernatural
• Teori Hippocratik
• Teori miasma
• Teori Kontagion
• Teori Germ (sebab ditunjukkan melalui postulat
Henle – Koch)
• Teori Epidemiologik klasik
• Teori multikausal dan jaring penyebab (sebab
ditunjukkan melalui postulat Hill)
Teori Germ (kuman) (2)
• Sebab ditunjukkan oleh postulat Henle – Koch
(kadang-kadang disebut “determinisme murni”
• Postulat Henle-Koch
▪ Agen harus ada pada setiap kasus penyakit
• Postulate Koch :
kuman penyebab penyakit
• Kelemahan dari postulate Koch ialah tidak
dapat diterapkan pada semua penyakit
Host
Agent Environment
Teori epidemiologik klasik
• Digambarkan dengan Triad epidemiologik
• Penyakit adalah hasil dari kekuatan dalam
suatu sistem dinamik yang terdiri dari
– Agen infeksi Agen
– Manusia (host)
– Lingkungan

GENERAL MODEL OF CAUSATION
(CAUSAL PIES)
BY KJ ROTHMAN
Sufficient cause:
• A set of conditions without any one of
which the disease would not have
occurred. (This is one whole pie.)
Causation slide # 21
GENERAL MODEL OF CAUSATION
(CAUSAL PIES)
BY KJ ROTHMAN
Component cause:
• Any one of the set of conditions which are
necessary for the completion of a sufficient
cause. (This is a piece of the pie.)

GENERAL MODEL OF CAUSATION
(CAUSAL PIES)
BY KJ ROTHMAN
Necessary cause:
• A component cause that is a member
of every sufficient cause.

Types of Causes
• Necessary – found
in all cases
• Contributing
–needed in some
cases
• Sufficient–a
constellation of
causes that makes
disease inevitable
Gerstman Chapter 2 28
Causal Pie Model
• Causal complement : faktor yg melengkapi
mekanisme kausal tertentu shg menjadi
sufficient
• Contoh causal complement
1. Untk A ; adl (B+C) dan D
2. Untk B ; adl (A+C)
3. Untk D ; adl A
Causal Complement
• Causal complement = the
set of factors that
completes a sufficient
mechanism
• Example: tuberculosis
– Necessary agent
Mycobacterium
tuberculosis
– Causal complement
“Susceptibility”
Gerstman Chapter 2 30
Understanding Causality
1. Necessary factor
2. Sufficient factor
Jenis penyebab
Pemapar E
Necessary Sufficient
-------------------------------------------------------
1. + +
2. + -
3. - +
4 - -
Types of Causal Relationships
• Necessary and sufficient – without the factor, disease never develops
– With the factor, disease always develops (this situation rarely occurs)
• Necessary but not sufficient – the factor in and of itself is not enough
to cause disease
– Multiple factors are required, usually in a specific temporal sequence
(such as carcinogenesis)
• Sufficient but not necessary – the factor alone can cause disease, but
so can other factors in its absence
– Benzene or radiation can cause leukemia without the presence of the
other
FAKTOR A DISEASE
Faktor A
• dibutuhkan 🡪 necessary
• sudah cukup 🡪 sufficient
• Jarang ada
Four types of causal factors
FAKTOR B
DISEASE
FAKTOR C
Initiation
+
Latent Period Cancer
+
Promoter
SUFFICIENT BUT NOT NECESSARY
• masing-masing faktor dapat menimbulkan penyakit yang sama
• Setiap faktor dapat bertindak sendiri-sendiri sebagai penyebab
penyakit
• Contoh :
penyakit (not necessary)
FAKTOR A
DISEASE
FAKTOR B
FAKTOR C
Sufficient tapi tak necessary
• Bila E adl sufficient tapi tak necessary utk
D 🡪 E selalu menimbulkan D, tetapi D
dapat terjadi tanpa E
• Contoh: Down’s syndrom mengakibatkan
retardasi mental, yg dapat diakibatkan oleh
berbagai faktor perinatal lain
Four types of causal factors
Ionizing Radiation
or
Benzene Leukemia
or
Electromagnetic
Fields?
NEITHER SUFFICIENT NOR NECESSARY
• Merupakan hubungan yang kompleks
• Representative untuk menggambarkan hubungan kausal
penyakit-penyakit kronis
FAKTOR A FAKTOR D
+
FAKTOR B or FAKTOR E
DISEASE
FAKTOR C FAKTOR F
Tak sufficient dan tak necessary
• Bila E tak necessary dan tak sufficient utk D
🡪 E dan D tak selalu ada bersama-sama
• Contoh:
Status gizi dalam kejadian penyakit menular
Four types of causal factors
Smoking + Cholesterol
and/or
and/or
Stress + Obesity
