AFFIDAVIT OF SUPPORT

I, Maria Mejia, of legal age, Filipino, married to Jose Mejia, and presently residing at 9
Caniongan, Cebu City, after being duly sworn to in accordance with law do hereby depose and
say:

1) That I am the mother of Ma. Victoria Mejia, born on December 3, 1977;

2) That I am giving my consent for my daughter to travel abroad;
3) That I am sponsoring her trip and financially capable to support her stay abroad;
4) That I guarantee that she will not violate any existing laws of the country of destination
(Florida, USA), and;
5) That I am executing this affidavit in connection with my daughter’s travel abroad.
