In 2019, the Corona virus Disease (COVID – 19) was declared as a global health crisis and the
Philippines was put in a state of public health emergency in 2020 in order to mitigate the
transmission of the disease.

In the meantime, to ensure continuity of learning, the Commission on Higher Education (CHEd)
mandated higher education institutions (HEI’s) to shift to flexible learning. CHEd Memorandum
Order (CMO) No. 4, Series of 2020 on “Guidelines on the Implementation of Flexible Learning” was
issued for AY 2020-2021.

Flexible learning currently is the most appropriate and safest learning approach during this
pandemic. However, there are certain STCW ’78 competencies in Maritime Programs where face to
face instruction would be necessary. As such, a well-planned, gradual and safe re-opening of
campuses would be essential, following strict protocols and guidelines as set forth by the CHED,
IATF, DOH and in collaboration with the local government units.

In a Memorandum from the Executive Secretary dated September 2021, the President has approved
the recommendation of the IATF in its Resolution No. 134-A (s. 2021) to allow the conduct of limited
face-to-face classes for the Marine Transportation and Marine Engineering programs of HEI’s in
Modified General Community Quarantine areas.

Cognizant of the need and based on the survey conducted among maritime students wherein a
significant numbers manifested willingness to undergo limited face to face instruction for their
laboratory components. The University has embarked on retrofitting its facilities as well as crafted a
recalibrated learning continuity plan.

As a 2rd year maritime student of the Our Lady of Fatima University – College of Maritime Education,
I have been oriented and duly informed about the face to face guidelines, requirements and
protocols of the University.

As such I voluntarily give this informed consent and signify my willingness to participate in the
limited face to face laboratory class and abide by all the health protocols as mandated by the
University.
