LOAD TEST CERTIFICATION

This is to certify that the Passenger Elevator (SMR) with capacity of 1350kgs, of

CORO HOTEL - MAKATI located in # 8436 Kalayaan Ave. Corner Mercado, Makati

City, Philippines is mechanically safe and sound.

This certification is issued based on the on the load testing assessment being conducted

last April 25, 2022.

It is assumed that the said elevator was been built in according to engineering

design and specification and duly supervised by a licensed Professional Mechanical

Engineer. Any deviation necessary during the occupancy of the building a construction

works shall properly approved by the Professional Mechanical Engineer.

This certification is issued upon the request of the owner for the issuance of permit

purposes only.
