AFFIDAVIT OF UNDERTAKING

I, ___________________________, of legal age, single/_________, Filipino,

a resident of ____________________________ and with business address at
___________________________ after having been subscribed to an oath, depose and
say:

1. That I am ______________________________ currently teaching at

__________________________________. That I am the constituted end-user of a
Laptop Asus ExpertBook B1500C with serial no. ________________ and external
Toshiba 1TB HDD with a Serial No.________________.

2. That I understand that I am executing this affidavit to commit to my duties and

obligations as an end-user/custodian of the above-mentioned device; to wit:

a. That the City of Pasig is lending to me the above-described property;

b. That I impose upon myself the responsibility of taking care of the property
with the same diligence I exercise over things that I legally own;

c. That as a borrower; I shall be responsible for its maintenance while the

same is under my custody;

d. That I shall endeavor to assure that the device lent shall be exclusively
utilized for educational purposes;

e. That I shall endeavor to assure that the device lent shall be kept in a good
and working condition on the date of its return to the City of Pasig; such
date shall be on ______________________;

f. That I shall not sell, transfer, assign or in any manner dispose of the device
lent, to any other person and that the said device shall only be used solely
by me;

g. That in case of loss; I shall immediately notify the City Government of

Pasig, through the Asset Management Division of the Pasig City Office of
General Services of the City of Pasig; custodian of the device lent and that
I should be liable for the shame should it be found out that it has been lost
through my fault.

h. That I understand that I shall be personally liable, if through my fault or

negligence; the thing is lost, damaged or has deteriorated; further;

i. That in case I have, in my possession and custody a laptop, other than the
one described above; I shall surrender the same to the City Government of
Pasig, through the Asset Management Division of the Pasig City Office of
General Services, and
 j. That I understand, that, the act of returning the previously issued laptop to
me is a condition precedent to the issuance of (description of the laptop);

3. That I understand that the failure to comply with any of the provisions of the
above-mentioned shall result to the automatic return of the device, and may be a
ground for the institution of any action or claim against me;

4. That I shall abide by all rules, regulations, orders or issuances relative to the
device;

5. I likewise DECLARE and ASSUME full and complete responsibility for all
consequences which may arise in connection with the use of the device lent and for
the safekeeping thereof;
