Introduction
Pharmaceuticals Fund and Supply Agency (PFSA) is mandated to procure, store and distribute
quality-assured, affordable pharmaceuticals in a continuous manner to all public health facilities
and build the capacity of health facilities in the proper management use of pharmaceuticals. The
Agency has been working hard to execute the responsibilities entrusted on it by the public.
Capacity of the workforce at all levels of the supply chain system has built through series of in-
service trainings in selection, quantification, inventory management, warehouse management and
proper use of pharmaceuticals organized by the Agency in collaboration with development
partners. Since its establishment, the procurement, storage and distribution capacity of the
Agency is showing significant increase from time to time.

As a result of the effort, health facilities are developing their own medicines list based on their
local morbidity pattern. The list serves as a basis for quantification and guides the overall supply
of medicines in the facility. The Integrated Pharmaceutical Logistic System (IPLS) being
implemented at health facilities has enabled the availability of logistics data necessary for the
quantification of pharmaceuticals for the national pooled procurement by the Agency.

Quantification is the key and initial step in availing needed pharmaceuticals at service delivery
points. The Agency manages both Program and Revolving Drug Fund (RDF) pharmaceuticals.
As the quantification should be need-based, health facilities are the source of data for the
national quantification. Unlike program pharmaceutical which is done at national level, the
quantification of RDF pharmaceuticals is done at health facilities and aggregated at the Agency's
branch and then central level to come up with the quantification data for the national
procurement. Accordingly, the Agency undertakes quantification of RDF pharmaceuticals on
annual basis by collecting quantification data from public health facilities through its branches.

This document contains the quantification of RDF pharmaceuticals done for the years 2009-2011
E.C and the final requirement to be procured for the year 2009 E.C (Annex. The quantification
included medicines, medical supplies, and laboratory chemicals and reagents that are availed
through RDF. It describes how quantification data were collected from health facilities,
aggregated at branches and center level, the basic assumptions made, strengths, weaknesses, and
outputs of the process. Quantification data was collected from a total of 2182 health facilities

(174 hospitals and 2008 health centers) which covers about 62% of the public health facilities
(86% of hospitals and 61% of health centers). Adjustments for stock on hand, service expansion,
private sector consumption, and related issues were done at respective hubs. National level
requirement of the year 2009 E.C. was prepared to guide supply planning and procurement.
Projection was made for the years 2010 and 2011 E.C. based on the 2009 forecast and
consumption pattern of previous years.

1. Scope of the quantification

The quantification is national and includes essential pharmaceuticals (medicines, Medical


Supplies, Medical Equipments and Laboratory Products) that are availed through RDF mainly
for public health facilities for the coming three years (2009-2011 E.C).

2. Objective

The main objective of the quantification exercise was to determine the national pharmaceutical
requirement managed through the RDF for the years 2009 to 2011 which will be used for supply
planning and resource mobilization for the procurement at national level.

3. Methodology

Technical team composed of staffs with various technical expertises in the area of medicines,
laboratory reagents, and medical supplies was established at central level under the Forecasting
and Capacity Building Directorate to work on the overall quantification process.

The quantification team followed a structured approach which included planning, preparation,
data collection, analysis, review, reconciliations, cost estimation and supply planning. The
national forecasts were developed using consumption data collected from selected health
facilities through 12 of the Agency's branches. The preliminary forecasts aggregated and
adjusted at branch and then central level was then presented at a technical meeting for further
discussion and consensus. Recommendations of the technical meeting were incorporated to

arrive at the final forecast presented in this document. Forecasts of the previous two years (2007
and 2008 E.C.) and the forecasts done for 2007-2010 was reviewed to make the necessary
adjustments to come up with the final forecast. Projection was made for the year 2010 and 2011
E.C. based on the 2009 forecast and consumption pattern of previous years.

Assumptions for quantification at Hub and Central Level


The following assumptions were made in undertaking the quantification exercise:
 The number of health facilities selected from each category is enough to represent the
total number health facilities found in the country.
 Health facilities for which extrapolation is made have similar consumption pattern to
those of the respective categories of health facilities that submitted their demand.
 The consumption pattern and price of pharmaceuticals will not significantly change in
the years to come.
 The updated AMC is reliable
 Demand will increase as a result of the launching of Social Health Insurance &
Community Health Insurance.
 Oromia & Amhara RHBs will continue the credit agreement with PFSA for pooled
procurement for selected health centers.
 Private health facilities will take up to 15-20 of the forecasted amount
 New hospitals under construction will being service provision soon.
 There will be sufficient budget to procure the pharmaceuticals forecasted
 Stock on transit will be cleared and ready for distribution at the end of Megabit 2008
E.C.
 Stock under the tender process will be delivered at the beginning of Hamle 2008 E.C.

4. Source of Information

The following data sources are utilized for the quantification.

 The forecast data sent by selected health facilities that is aggregated and adjusted at hubs

5. Quantification Process

Preparatory phase:

The Forecasting Team established at central level revised the list of pharmaceuticals to be
quantified and the relevant formats based on the level of health facilities. These documents were
communicated officially to hubs. Hubs accordingly established a quantification team composed
of hub manager, forecasting and capacity building officers, stock and distribution officers, and
partners. Some hubs have included Regional Health Bureaus (RHBs) and Zonal Health
Departments (ZHDs) in their quantification teams. Some of the hubs collected quantification
data from all health facilities under their catchment whereas others collected data from sampled
health facilities and extrapolated to others by level of health facility.

The quantification template was officially communicated to health facilities directly or through
RHBs or ZHDs. Some RHBs and ZHDs were able to orient health facilities on how to conduct
the quantification on events prepared for other purposes during the quantification period. Some
of the hubs like Adama and Mekele have also prepared and send quantification job aid for health
facilities to be used as a guide in the quantification process. The hub level quantification team in
collaboration with the pharmacy unit of the respective RHB had routinely supported health
facilities on how they will track the required essential data items from the LMIS records, before
communicating the quantification template to facilities. And then the information was
communicated to respective zonal health departments, Woreda Health Offices and facilities
through different means such as email, meetings and letters. In close collaboration with
administrative units and partners, hubs strived to provide technical support to selected facilities
on how to conduct need based forecast and some supportive documents were also provided to
facilities during the site support. Some of the Woreda Health Office were able to collect and

aggregate forecast data of the health centers under them and send the compiled data to respective
hubs.

Data collection and analysis phase:

After aggregating and organizing the whole data at branch level, item wise analysis and
adjustments were done at hub based on the assumptions set, experts opinion and quantification
data consumption trends. The final quantification result was approved by the hub manager and
mailed to the central quantification team. Official hard copy was also sent to the head office.

Quantification data was collected from a total of 2182 health facilities (62.3% of the total
number of public health facilities) consisting of 174 hospitals (86% of the total number of public
hospitals) and 2008 health centers (61% of total number of health centers available). Hospitals
were categorized into referral, general and primary and health centers categorized into high,
medium and low volume. Aggregation of data at hub level was made per health facility level and
extrapolation made for each health facility level prior to determining the total amount of each
pharmaceutical required. Table 1 shows the proportion of health facilities covered in the
quantification.

Following the data collection and aggregation phase, PFSA with financial support from Clinton
Health Access Initiative (CHAI) organized a 2009 Budget year RDF pharmaceuticals
quantification exercise workshop from January 22-25, 20165 at Executive Hotel, Adama. The
Workshop was aimed at reviewing the aggregated national forecast data jointly by experts from
head office and branches who were involved in the quantification process plus relevant experts
from Procurement and Distribution Directorates of the Agency. A total of 49 experts drawn from
the Agency’s Core Processes (Forecasting and Capacity Building, Procurement, Storage and
Distribution Directorates) , and Branches (Managers and Forecasting and Capacity Building
Coordinators) participated in the quantification workshop. The workshop gave an opportunity to

discuss on the overall quantification process, the assumptions made, the limitations and way
forward at both central and hub level. It was also possible to identify share best experiences
among hubs which will be useful for future quantification exercise. The active involvement of
RHBs, ZHDs and WdHos, developing and distributing quantification job aids for health
facilities, and encouraging RHBs, ZHDs and WdHos to own the quantification process were
some of the good experiences identified. Following the experience sharing session, participants
from each branch (assisted by one or more members of the central forecasting team) sat together
and made final review on the forecast data of their branch. After the necessary adjustments were
made on the workshop by each of the branches, the final data was forwarded to the central
Inventory Control and Forecasting Team of the Forecasting and Capacity Building Directorate
where supply planning for the 2009 E.C forecast and projections for 2010 and 2011 E.C. was
done (Annex I). The national procurement will be guided by this forecast data.

6. Strengths of the Quantification Process

 Using the feedbacks from 2008 quantification workshop organized by the head office

 Integrating the onsite support on quantification with ART/TB regimen data collection
enable to cost saving

 Use of quantification job aid by some hubs helped health facilities

 Strong collaboration with the branch’s stock and distribution team

 Onsite support and continuous follow up by phone to the health facilities

 Relatively higher coverage of health facilities than the previous year

 Getting the technical support laboratory professionals from hospitals in quantifying


laboratory products

 Early initiation of the quantification process

 Better involvement of RHBs, ZHDs and WoHOs in the quantification process

 Well-designed and simplified quantification tool

 Strong Commitment from staff at branch FCB and Logistics Focal persons of Regions &
zones

 Better data quality as compared to previous years

 The availability of fund for onsite support

 Better workforce at PFSA Hubs

7. Limitations of the Quantification Process

 Absence of functional DTCs at most of health facilities to facilitate and oversee the
quantification process

 Low commitment of pharmacy professionals less attention given by health facility


management at some health facilities

 Shortage of pharmacy work force at health facilities and administrative levels (RHB,
ZHD, WoHO)

 Lack of skill on Microsoft-Excel operations at health facilities

 Failure of some health facilities to submit their demand and Data quality problem

 Delay in submitting the quantification result to hubs.

 Inadequate involvement of RHBs, ZHDs and WoHO in the quantification process.

 Health facilities in some zones of Oromia Region not willing to submit their demand due
to the establishment of another supplier in the region.

 The quantification tool doesn’t allow considering expiry date of the stock on hand

 Quantified amount not reconciled with expected amount of budget at some health
facilities

 Difficulty of using the quantification tool at health facilities.

 Over quantification by some health facilities.

 Shortage of pharmacy workforce at some health facilities

8. Way Forward
 Starting the quantification exercise immediately after the annual physical
inventory to have adequate time

 FMOH, RHBs, ZHDs, and WoHOs should play leadership role in the
quantification process for health facilities under them

 Adequate number of pharmacy professionals should be available at health


facilities, RHB, ZHDs and WoHOs

 Feedback should be provided to health facilities regarding the data quality for
future improvement

 DTCs should be established and made functional at health facilities

 There should be regular supportive supervision and follow-up on IPLS


implementation at health facilities

 Quantification SOPs should be developed for RDF pharmaceuticals as part of the


IPLS SOPs or separately so as to standardize the process and come up with
quality data

 Quantification training on RDF pharmaceuticals should be provided to PFSA,


health facility, RHB, ZHD and WoHO staff as part of the basic IPLS training or
separately.

 Orientation on the quantification of RDF pharmaceuticals should be given to


relevant managers at RHB, ZHD, WoHO and Health Facility

 The description and unit of issue of pharmaceuticals has to be standardized in


both HCMIS, reporting formats & the national quantification list.

 Capacity of PFSA and health facility staff should be built in the area of medical
supplies, laboratory reagents & chemicals.

 The quantification tool should have space to account expiry date of stock on hand
while calculating net requirement

 PFSA and health facilities should strictly adhere to the annual quantification data
in requesting and supplying RDF pharmaceuticals

 Instead of putting the “Other products list” at the bottom of every category, it is
better to have separate sheet for these items for ease of aggregation

 Adequate budget and vehicle should be available at hubs to undertake the


quantification

 Discuss with Oromia RHB regarding the supply of RDF pharmaceuticals to health
facilities in the Region in relation to the supplier established in the Region.

 Gap filling DTC & IPLS training should be provided

 On the quantification one year consumption data should be used

 PFSA should avail RDF pharmaceuticals as per the demand of health facilities

 Capacity of other professionals working on pharmaceuticals management at


health facilities should be built

 Create a mechanism that can make health facilities who fail to send their demand
accountable for their action.

 Health facilities should work hard on improving their data quality

 Quantification RDF Pharmaceuticals should be one of the key activities in the


annual action plans of health facilities

 Health facilities should involve their DTC in the quantification process

 There should be briefing/orientation for all concerned prior to the actual
