You are employed at Microsoft" and support Microsoft’s cloud-based services and will have access to customer
data. (“Engagement”).

This form is being provided to you because Microsoft would like to obtain your clear, free-will, unequivocal and
prior informed consent for a background screening (“Background Screening”) in connection with this
Engagement. Microsoft also wishes to obtain, subject to your clear, free-will, unequivocal and prior consent,
additional periodic Background Screening during the course of the Engagement. If you decide not to consent to
this Background Screening it will not be possible to make use of your services in connection with this
Engagement.

Purpose of Background Screening

Our Background Screening is undertaken due to the fact that Microsoft has internal restrictions about staff vetting
of personnel deployed to cloud-based services, in particular those with access to customer data. This Background
Screening may be used, to an extent permitted under applicable substantive (e.g. data protection, employment
law) and procedural laws, in determining your suitability for the Engagement.

Content of Background Screening

The Background Screening typically includes, insofar as it is relevant for the respective Engagement, information
relating to:

- Criminal Record Check (where lawfully permitted)

- Identification check
- Global Watchlist check

The sources of such information may, insofar as permitted by applicable laws and available to us, include, but
are not limited to, criminal records, court records and various publicly available official governmental/non-
governmental data sources, as well as social networks or other publicly available information found online. The
checks may also include a check of the US Office of Foreign Assets Control list, the US Bureau of Industry &
Security Denied Persons list, and the US Directorate of Defense Trade Controls Parties Debarred list.

Process of Background Screening

Microsoft will usually use a third party service provider (“Screening Vendor”) to assist with the conduct of
searches of these sources and provide a background screening report(s) to us. Any background screening
information obtained by Microsoft may be communicated among Microsoft's affiliates or subsidiaries for the
purpose of ensuring the accuracy of the information collected and your professional qualifications for the
position. Microsoft and some of these affiliates or subsidiaries, as well as the Screening Vendor, are located in
or may process your personal information in, jurisdictions (such as but not limited to the USA) which may not
ensure the same or adequate level of protection or rights for your personal information as you enjoy in the
country in which this information is collected or which you are otherwise familiar with. If you do not want
Microsoft to communicate such information among its affiliates or subsidiaries, you must notify the Microsoft
or your HR Manager mmediately.

The Screening Vendor is located in the United Kingdom and processes the data in countries with the same or
adequate level of protection or rights for your personal information as you enjoy in the country in which this
information is collected. Microsoft and some of its affiliated companies, if necessary for a specific deployment,
may receive and process the provided personal data and information and the results of the Background Screening.
Such affiliated companies are located in jurisdictions (such as but not limited to the USA) which may ensure the
same or adequate level of protection or rights for your personal information as you enjoy in the country in which
this information is collected. Microsoft will ensure that your data is in any case sufficiently protected against
unauthorized access.

AUTHORIZATION

I have read and understand this notice and I authorize (and so explicitly give consent for) Microsoft and the
Screening Vendor acting on behalf of Microsoft, to obtain a Background Screening report and to collect
thereby, use, retain and share information about me in conjunction with my cloud-based services engagement
for the purposes specified above and to share such report and respective information about me with any of
Microsoft's affiliates for the purposes specified above and the fact of an unsuccessful Background Screening
(without detailed findings). This consent is given having been informed of the type and nature of the personal
information being collected and that Microsoft, and the Screening Vendor and Microsoft’s affiliates are located
in or may process my personal information in jurisdictions which ensure the same or adequate levels of
protection or rights in respect of my personal information that I have in the country in which the data was
collected and so includes consent to such transfers. I understand that this consent will apply during the course
of my cloud-based services engagement, should I obtain such engagement, unless I revoke or cancel my
authorization by sending a signed letter or statement to Microsoft at any time, stating that I revoke my consent
and no longer allow Microsoft to obtain Background Screening reports or (again as applicable) to share them
with its affiliates. I understand that if I revoke or cancel my authorization this may affect my engagement with
Microsoft’s cloud based services. I further understand and agree that all of the information evaluated by the
Screening Vendor or otherwise disclosed to Microsoft by me may be utilized for the purpose of obtaining
Background Screening information. I confirm that all such information provided by me is true and correct, and
I understand and agree that Microsoft and the Screening Vendor may verify the truth of this information. I
authorize without reservation any party or agency contacted by Microsoft or Hireright to furnish the above-
mentioned information.
