We, the undersigned 4th year Civil Engineering students of Cagayan State University – Carig
Campus, are currently conducting a research study titled “RISKS AND ACCIDENTS: ITS
RELATIONSHIP TO BUILDING CONSTRUCTION ACTIVITY AND ACTUAL
CONDITION OF THE SITE” as part of the requirement for the completion of our degree
program.

In this regard, we would like to request data on accident reports that occurred in construction
companies of Region 2 for the year 2017-2022. Rest assured that the information provided
