After the implementation of the
Enhanced Community
Quarantine, Local Government
Units in Metro Manila scheduled
the simultaneous vaccination of
the A1, A2, and A3 sectors. A1
refers to Medical Frontliners, A2
to Senior Citizens, while A3 covers
Persons with Comorbidities.
WHERE TO REGISTER
FREE COVID-19 VACCINE

• To ensure a smooth rollout of COVID-19 vaccines,


several local government units (LGUs) in NCR +
bubble have launched their online registration
platforms.

• Most online registration forms require the sector


from which the registrant belongs to, their address,
identification card numbers, medical histories, and
previous diagnosis for COVID-19, if any.

• Most schedules are being provided once you have


completed the registration.

• CITY OF MANILA
Go to the http://manilacovid19vaccine.com and click the “register or
reserve” button. Fill up the registration form and click “validate,” enter the one-
time password that will be sent through your cell phone number and click
register.
Then click the “I agree” button and affix your signature on the waiver page.

• MAKATI CITY
Visit https://www.covid19vaccine.safemakati.com to register. Upon
visiting the website, you will be asked about the beneficiary sector you belong
to and as well as your ID number.
Provide the required details to complete the registration.
• LAS PIÑAS CITY
Visit and access this link > Las Pinas Google Form for the vaccination registration. Upon
clicking the link, you will be asked to identify the sector you belong to, contact number,
address, medical history, COVID-19 infection history, and other details.
Registrants will likewise be asked for a 1×1 picture in white background and a photo of your
government-issued ID. Click “submit” after filling out all the required fields.

• MUNTINLUPA CITY
Visit https://vaccine.muntinlupacity.gov.ph/muncovac/ to access the COVID-19 vaccine pre-
registration form of Muntinlupa City. In the webpage, you will need to input your personal
information, allergies, and medical history.

• CALOOCAN CITY
The pre-registration form of Caloocan City can be accessed through this link > Caloocan
Google Form. However, as of March 29, the website says that the form is no longer
accepting responses.
• NAVOTAS CITY
The pre-registration form for Navotas City can be accessed through
https://covax.navotas.gov.ph/ . Input the required details and fill out
the consent form then click “Submit”

• PARAÑAQUE CITY
Visit https://m.oneparanaque.info to register. Sign up by providing
your email and password or by linking your Facebook account. You
will then be asked to provide your mobile number and to upload your
photo.
Upon signing up, click “register now” on the next webpage that will
appear, input the necessary details, then click “submit”.

• SAN JUAN CITY


Visit http://120.28.52.92:8002/vaccine_registration/public/ to register
for San Juan City’s vaccination drive. Upload a photo of your valid ID
and 1×1 picture, and fill out the required details. Click “submit” upon
completing the form

• VALENZUELA CITY
Click this link
lhttps://valtrace.appcase.net/Register/Login?vaccine=true to go to the
registration form. Your log-in details that will be used in registration
are the same as those you used for the ValTrace QR Code. Fill out the
required fields then submit the completed form.
• MANDALUYONG CITY
The registration form for Mandaluyong City’s vaccination can be
accessed through this www.mandaluyong.gov.ph/vaccine/ The website
will require your personal details and your photo. Click “register” upon
completing the form.
A notice showing you have successfully registered for the vaccination
will appear. Click the link below the same page to get your waiver and
immunization card.

• PASIG CITY
The LGU would use its “PasigPass” app, the city’s QR Code contact
tracing solution, for the COVID-19 vaccination program instead of
asking residents to register again for the inoculation drive. If you have
not signed up yet, here’s the link: https://pasigpass.pasigcity.gov.ph/

• TAGUIG CITY
The LGU’s Taguig Registry for Assessment and City Engagements, which
can be accessed through https://tracetaguig.com/taguig-login, has
expended from being a contact tracing tool into a registry for other
services such as vaccination, other health benefits, and social services.
QUEZON CITY
There are two ways to register for COVID-19 vaccination in
Quezon City, according to a Facebook post of the city
government:

1. The first is through online self-booking, wherein you will be


required to sign up as a patient at the EZConsult website :
https://app.ezconsult.io/signup?userType=Patient

QUEZON CITY
The EZConsult App can also be downloaded in the App Store or
Google Play.
According to an advisory from the QC government, “your
account will be under review until you are eligible based on the
DOH priority list.”
QCitizen ID will not be required in registering through
EZConsult, but your email address and mobile number will be
needed for registration.

• CAVITE
The provincial government of Cavite has ordered three million doses of
COVID-19 vaccine from US biotech firm Novavax to beef up the inoculation
program of the national government. Governor Jonvic Remulla said the
Novavax vaccines will be alloted for residents aged 18 to 59. The vaccines
are expected to be delivered as early as July and not later than December.

ONLINE REGISTRATION STEPS:
1. For those persons with comorbidity (A3 Category), prepare a
valid ID with supporting documents.
