Residential Service Agreement

SERVICE AGREEMENT

CTC will provide to you, the Member, the agreed upon internet service for a 24-month period without raising monthly rates
(CTC Price Lock). In return, you agree to pay the agreed upon internet service rate stated on the service order, or the
current advertised lowest internet service rate, not to be below $50, for a period of 24 months. The 24-month term will
expire 24 monthly billing cycles from the date CTC installs internet equipment and services at your residence (“Start Date”).
The CTC Price Lock does not include the cost of installation and equipment, taxes, or late fees, or costs for other services
outside of the internet service that you may order.

After the CTC Price Lock guarantee term expires, unless you otherwise notify us, CTC will continue to provide internet
services to you on a month-to-month basis at our then-current standard monthly prices for such services. After the
24-month period, you must give CTC written notice of termination. Services will be terminated at the end of the next billing
cycle after receipt by CTC of your notice to terminate.

TERMINATION AND EARLY TERMINATION FEE

You may terminate this 24-month agreement for the internet service at any time by calling CTC at 218-454-1234; however,
you may be assessed an early termination penalty of up to 100% of the remaining months of your contract at the current
advertised lowest internet service rate, not to be below $50. In addition, any applicable late fees, equipment, taxes, and
discounts or credits that were applied during your installation or monthly services may be applied. This may include costs
that CTC has incurred during the installation of fiber, equipment, or services to your location.

In the event that either CTC cannot deliver, or the customer is delayed from paying due to any cause beyond either party’s
reasonable control, (for example, an act of nature (ex. catastrophe), act of man (ex. vandalism), or an act of Government
(ex. war), or), such an event will not constitute grounds for default. Reasonable time for remedy will be adjusted equitably
between you and CTC.

Equipment supplied by CTC must be returned by you immediately upon termination. In the event you fail to return CTC
equipment, CTC will invoice you for the fair market value of such equipment.

OTHER TERMS AND RESTRICTIONS

This offer is subject to your agreement to CTC’s standard service terms and conditions for services. It is also subject to the
availability of service at your residence and is available only to persons who have no outstanding obligations to CTC. This
offer is also subject to a positive credit check. Deposits may be required to secure services. This offer is not available to
seasonal service customers or business customers. Services will meet industry standards. CTC will maintain its facilities and
equipment used to provide the Services set forth in its policies and procedures, at no additional charge to Customer. CTC
will maintain in good repair and operation fiber lines to your optical network terminal (“ONT”) on your property. You are
responsible for the wiring and facilities from the ONT into your property (“Customer’s Facilities”). In cases where work or
service calls result from failure or malfunction in, or improper operation of your Facilities and/or equipment, you will
reimburse CTC for the cost of the required maintenance at CTC's standard time and material rate plus any taxes imposed
upon CTC related to such maintenance.
