MEMORANDUM OF AGREEMENT

This Memorandum of Agreement is made and entered into by and


between:

educational institution duly recognized and existing under the
laws of the Philippines, through the Office of Career Services,
with main campus address at Cabambangan, Bacolor, Pampanga,

WHEREAS, the FIRST PARTY is a state university U which provides,

WHEREAS, the FIRST PARTY allowed its students to undergo


Internship, whereby proposal was accepted and approved subject to
terms and condition provided in this Memorandum of Agreement;

WHEREAS, both parties are cognizant of the need for cooperative


endeavor and are fully aware of the benefits that could be met in
this undertaking;

NOW, THEREFORE, for and in consideration of the above premises,


the parties hereunto have jointly agreed to implement a scheme
designed to achieve their common objective as follows:

A. The FIRST PARTY (DHVSU Main and its Extension Campuses) shall:
1. Provide officially enrolled and qualified students to
undergo Internship for required number of hours to the
SECOND PARTY. (See attached List of Certified Student
Interns);

2. Designate Student Internship Program in the Philippines
(SIPP) coordinator to monitor attendance, follow-up on
student interns’ performance and coordinate with the HTE
regarding various activities/requirements under this

B. The SECOND PARTY shall: Affairs
1. Serve as the Host Training Establishment and (HTE) for the

2. Only accept and accommodate student interns with complete

endorsement papers required in their internship;

3. Provide student interns an Orientation entativ regarding the

agency/industry policies, rules and regulations and its
operation before the start of the internship;
4. Provide Supervised Applied Learning Experience to student
interns;
5. Station student interns strictly within the company
jurisdiction via online platform unless otherwise
Commission on Higher Education (CHED), Department of Health
(DOH), and Inter-Agency Task Force (IATF) on COVID-19
already permitted the face-to-face HTE conduct of Student
Internship Program in the Philippines; Author
6. Enforce and strictly adhere to the IATF ized Health Protocol

physically report to the station due to e urgency and the
nature of the work, such as but not limited to submissions
of reports and technical assistance;
7. Render eight (8) hours daily of Internship. (In case there
is a need for an extended hour, student interns can only do
overtime for a maximum of two (2) hours);
8. Not require the student interns to render night shift duty;
9. Assign Training coordinator/supervisor who will take charge
and supervise the student interns while undergoing
internship;
10. Allow the SIPP Coordinator of DHVSU to conduct
actual/virtual visit at least two (2) times in a month to
monitor the status of the student interns and the program;
11. Issue a Certificate of Completion and sealed Performance
Evaluation to student interns after serving the required
number of hours.

C. FURTHER, the following terms and conditions are likewise
included:
1. The SECOND PARTY (HTE) reserves the DOLO right to discontinue
the Internship of the student interns RES on reasonable

1.6 Involvement in violence, harassment or other form of
gross misconduct inside the company’s premises;
1.7 Any findings regarding the performance and decorum of
student interns shall be forwarded to the DHVSU Office
of Career Services or can be coursed through to the
assigned SIPP Coordinator for deliberations and
appropriate actions.
2. No dues will be collected from both DHVSU HTE and HTE as the

3. Both Parties shall ensure confidentiality

records/personal information in reference to Republic Act
No. 10173 also known as -“Data Privacy Act of 2012, its
Implementing Rules and Regulations related issuances and
applicable laws”. These records shall only be used for
internship, placement, and research purposes alone. The
Parties agree at all times during and after the term of
this MOA, to keep in strictest physical, organizational,
and technical measures and to take all reasonable
precautions to prevent its disclosure or misuse. Parties
and any of its officers, directors, employees, agents and
representatives shall hold all Confidential and Personal
Information disclosed to it by reason of this MOA
confidential and private and shall not disclose any/such
information to any party subject to legal obligations and
applicable laws. Parties agree not to use Confidential and
Personal Information for any purposes whatsoever other than
for the provisions of this MOA.

This agreement shall take effect immediately upon signing
hereof, and shall continue hereafter, either party may terminate
this agreement at any time by serving written notice to the other
party, giving thirty (30) days lead time before intended date of
termination.
