The following features are required on a prescription: ‘* Name of Patient ‘+ Date the prescription was issued ‘¢ The prescribed drugs by ~ Name of the drug Dosage or strength of drug Route of administration Frequency of administration Duration of treatment © Prescriber's Signature ‘© Prescriber's/ Institution's stamp We would lke to remind all members that the requirement of a copy of the valid prescription is a necessity and will faclitate the processing ofthe claim efficiently. Furthermore, its required by law if the member wishes to claim their tax credits for any shortfalls incurred. In the case of chronic or repeat medication, a copy of the prescription must be attached on first sue and will remain valid for the duration of the period stated by the doctor. Please ensure that all prescriptions for long term repeat medications are renewed atleast once every six months. All refunds should be submitted to the society within 3 months of purchase of medication. \f the refund is to the member the claim must be accompanied with a cash receipt.
