It is our first day of community service, and I am already getting ready
for our duty at 5 a.m. so that I am not late. To avoid unnecessary trips, I make sure to pack all of the necessary supplies in my CHN bag before leaving. I'll be riding a bus to the location designated for us. It's nice to start the day with some fresh air and time to put my mind at ease before the hectic day ahead of me. Our first community duty was in Barangay Lucao, where we were assigned to Purok 1. As we walked, looking for our target family, our feet led us to the Banan family, who live at 74 Purok 1 Brgy. Lucao, Dagupan City. Mrs. Jona Marie Banan was the one I spoke with. During the interview, I noticed that she and her children were both thin. They don’t eat meat because it’s expensive, but they do eat vegetables, and that is not enough. It makes me realize how important it is to value the food that is served on the table and not waste it because we are still fortunate to have enough healthy and delicious food, whereas some other people do not have enough food to eat and I believe they yearn for delicious and healthy foods. Her daughter has a medical history and was hospitalized when she was three months old, but now that she’s three years old, she’s okay and wasn’t hospitalized. Mrs. Banan's father has diabetes, and her mother has asthma. Mrs. Banan stated that they do not yet have their own home and that is also one of their priority needs. They are only staying with their mother-in-law. That is why they have an extended family. Her two children have not yet been immunized, but she and her husband have already received COVID-19 vaccinations. I also notice that the surroundings, due to a lack of space and the presence of mosquitoes, can cause diseases such as dengue. When we accomplish this duty, I have a buddy system, and I am with Charmaine Joy Chan and Kathleen Abrejal. I enjoy having the opportunity to interview clients in their homes and develop relationships with clients and their families. We work with clients to help them achieve their personal health goals at home. Cancino, Angela Ann M. October 20, 2022 BSN II-A MY DIARY This was our 2nd day of duty in the community setting and our last duty for the month of October. I didn’t come back to the Banan family because I find another client that has a case of asthma so I have a case to put in my nursing care plan. I am searching around Purok 1 at that time and finally, I found Mr. Ramon Santos, who lives at 105 Purok 1 Brgy. Lucao, Dagupan City. I asked him if I could interview him, and he agreed. He is 70 years old with four children, but they are already young enough. Still, they are an extended type of family, so Mr. Santos is accompanied by his children and grandchildren in their home. Sometimes he works as a tricycle driver. But he stops due to his asthma. He said that it is not that complicated because he can still manage his disease. Every time he eats any kind of seafood, that’s the time that he has difficulty breathing. He has an allergy to certain kinds of seafood, which triggers his asthma attacks. I observed that his left foot has atopic dermatitis or eczema due to eating "talaba" and that’s the time that it’s itchy. So, I encourage Tatay Santos to avoid foods that can trigger his asthma, as well as the environment he is living in. It should be clean because dust, air pollutants, flowers, and animals can trigger his asthma. He mentioned that he is now a widower because his wife died when she was 61 years old due to complications of asthma. He also mentioned that at around 50 years old, that’s the time that he's having signs and symptoms of asthma. Both parents of Tatay Santos have asthma. After the interview, I checked his vital signs. His body temperature was 35.8 degrees Celsius; his pulse rate was 81, his respiratory rate was 18, his blood pressure was 160/90, and his SpO2 level was 96. After checking his vital signs, there are six people who saw us and they ask if I can get their blood pressure. So, as a student nurse, it is important to check the health of people in order to identify if they have a disease to prevent and care for. When we accomplish this duty, I have a buddy system, and I am with Malaya Anne Vinluan and Athena Riotita. I enjoy having the opportunity to interview clients in their homes and develop relationships with clients and their families. We work with clients to help them achieve their personal health goals at home. After that, we go back to the barangay hall of Lucao to get the vital signs of a pregnant woman. We interviewed Ma’am Jean Bariqued and take her vital signs. Her body temperature was 36.5 degrees Celsius; her pulse rate was 118, her respiratory rate was 20, her blood pressure was 130/80, his SpO2 level was 98, her weight was 70.5 kg, and her height was 178 cm. Her birthday was on October 23, 2007. She was 15 years old only and it is her first time having a baby. Her last menstrual period was on February 8, 2022, and her Expected Date of Delivery is November 15, 2022. After that tiring day but a joyful day, we went home and have an online class in the afternoon.
