I do affirm and declare on oath that:

1. All my documents/credentials and certificates which were used in the determination of merit list
for the appointment against the above-mentioned post are genuine and correct and no document is
Fake/Tempered or Fabricated.
2. That all my documents were issued by the concerned institutions after observance of the required
formalities.
3. I further state that I am aware of the facts and nothing is being concealed by me.
4. I further affirm and declare that in case my document(s) are found fake or fabricated or tempered
by any means, the department has the right to withdraw my appointment order and I shall observe
that decision without any objections.
5. I further add that in case my document(s) are declared Fake/Bogus by any means and my
appointment is withdrawn by the department, I shall be bound to return all the financial benefits
received by me during service period.
6. I also declare that in any ambiguity, the decision of the Education Department would be final and
accepted by me.
7. That in any case of any disciplinary proceedings against me in the light of the above-mentioned
statements, I shall have no right to appeal and will not consult any court of law.
