Design details of the main, work activities involved and safety measures to be in place are then
submitted to the Indonesian Department of Mines for approval prior to start of mining operation.
The Department of Mines conduct mine inspections at regular basis.
All new mining activities are properly planned and implemented with the help of third party
consultants. The design details of the mine, work activities involved and safety measures to be in
place are then submitted to the Indonesian Department of Mines for approval prior to start of
mining operation. The Department of Mines conduct mine inspections at least twice a year.
Safety measures due to mining operations using heavy equipment units include:
Motto:
There is no soonest or most important activities without adequate times to do safely. (MTN)
Buma: B’Safe
- Location of warehouse
- Maximum Capacity of The warehouse
- Distance to other building and facilities
- Large truck/equipment (HD 785, CAT 777) 1 unit ABC 9 kg outside cabin
- Little truck (Hino, Isuzu, Nissan), Little Bus 1 unit ABC 5 kg inside cabin
- Large Bus 2 unit ABC 5 kg inside cabin

- Fuel Station 4 unit ABC 9 kg
- Fuel Truck/Lube Truck 2 unit ABC 9 kg outside cabin
- Generator house 1 unit ABC 9 kg (Outside)
- Main fuel tank hydrant (Water and foam)
- Check Pre-Inspection Form
a. Working on side wall (any examinations prior to begin work at high walls)
b. Any daily inspection to check pit wall angles
c. Position of truck during loading
4. Hazards Operation
4.1. Highwalls Operation
- Any rock
- Any side wall less than 50 degrees
- Highwall surface (structural stability – top and bottom)
- Ravelling along bench face
- Cracks
- Plane failure
- Wedge Failure
- Step-path Failure
- Toppling Failure
- Tension Cracks
- Scarps
- Abnormal water Flows
- Bulges or creep
- Rubble at the Toe
- Incident result that have ever been happened at haulage and what cause it
- Design (well-designed and constructed, haul roads can make the aggregate operation safer,
more productive, and cause less wear and tear on equipment)
- Length of haulage road
- Width of haulage road usually designed to be 3 times the width of the largest haul truck (for
two way traffic) with extra width employed on the curves.
- Distance between dump point and mining pit
- Usage of haulage road
- Haulage road condition (earth roads, chip seal, harded clay, land road)
- Intersection by other haul roads or by the regular road
- Contour of the haulage road (flat, straight, sinuous, undulating)
- Traffic sign
- Traffic Control Signs (keep right, one way, no left return, do not pass, sound horn, blasting,
turn off 2 way radios, etc.
- Road shoulder barriers/berms should be ¾ of the height of the larges tire on any vehicle
hauling on the road.
- Any sharp horizontal curves designed at the top or bottom of hills?
- Traffic condition (traffic density)
- Check profile of elevated haul road on site hill
- Check profile of elevated haul road on flat ground
Note:
Keep frequently used haul roads and other heavy traffic areas dry by elevating the surface to
facilitate runoff. Dryer road caused less wear and tear on tires and equipment and creates less
sediment-laden water that will require treatment.
Maintenance
(Such regular grading program that will minimize erosion, sediment build-up, noise and dust; to
ensure that potholes, washboarding are repaired immediately to minimize noise, dust and
equipment wear; spray water, calcium chloride or other approved dust suppressant on the road
surface; road may also require scarifying, sanding and resurfacing)

- Grounding, bonding
- Flammable liquids (storage, amount, safety)
Note:
Hazard identification process should begin by identifying al potential initiating events through
which harm could be realized and in particular:
